Starting therapy can feel daunting. Many people feel anxious about what to expect in their first therapy session, worry about saying the ‘right’ thing, or if they’ll feel judged. If this sounds familiar, you’re not alone. These feelings are normal, more so if you’ve never had counselling or psychotherapy before.
In this post I wanted to explain what usually happens in a first therapy session to help you feel more prepared, informed, and reassured.
A Space to Talk at Your Own Pace
The first therapy session is mainly about getting to know you and understanding what has brought you to therapy. There is no pressure to talk about everything straight away, and no expectation that you arrive knowing exactly what to say.
Some people come to therapy with a clear concern they want help with, such as anxiety, low mood, or stress and the impact this is having on their daily life. Others simply know that something doesn’t feel quite right. Both are completely okay. The session will move at a pace that feels safe and comfortable for you.
What the Therapist Might Ask
During your first session, you will be asked some questions to help build an understanding of your situation. This might include:
- What led you to seek therapy at this point
- How your difficulties are affecting your daily life
- Relevant background or experiences
- What you hope therapy might help with or focus on
You are always in control of what you choose to share. If something feels too difficult to talk about, you can say so. Therapy is a collaborative process, not an interrogation.
Common Worries About the First Therapy Session
Some common worries before a first therapy appointment include:
- “What if I don’t know what to say?” – That’s okay I will help guide the conversation.
- “Will I be judged?” – Therapy is a non-judgemental, confidential space.
- “What if I get upset?” – Emotional responses are okay and supported in therapy.
What the First Therapy Session Is — and Isn’t
It can be reassuring to know what therapy is not. The first session is not a test, not about being told what to do, and not about ‘fixing’ you — because you are not broken. Therapy is a safe and supportive space to explore your thoughts, feelings, and experiences with compassion and care.
What Happens After the First Session?
At the end of the session we will talk about what feels helpful going forward. This could include whether you’d like to continue with therapy or developing a plan for therapy including how sessions might be structured.
There is no obligation to commit immediately. Taking time to decide what feels right for you is an important part of the process.
